Molly 说,到现在resident 二年级了,用了两年的时间,更加能明白一个program喜欢
要什么样的resident了。 Molly 想更多的从program的方面跟大家聊聊如何将自己的
package更加完善。

Molly 背景:step one 230+, Ck 240+, CS:first pass, 目前OBGYN 二年级。

How to give constructive criticism?
Joseph Cooper, MD

Set the stage: Provide clear expectations. Sometimes this requires going the
extra mile and providing a written document rather than verbally setting
expectations at the beginning of a rotation/week on service. I have drafted
my own “expectations,” that I email to all learners prior to coming on
service. This step is key to providing useful feedback at the end of a
rotation, because the learner can’t say “well, I didn’t know I was
supposed to do XYZ.” Setting the stage also applies to the environment in
which feedback is given: in a timely manner, and one that is private,
protected, and confidential.

Ask open-ended questions: You can never go wrong with this. With patients
or with learners. This is a skill most of us accept as important, but often
fail to use correctly. Beginning a feedback session by allowing the learner
to self-reflect on his or her successes will usually allow for an easier
transition to discussion of areas where the learner needs improvement. A
simple introduction such as: “So, what did you think of the past week?” is
non-threatening, open-ended, and allows the learner to start and (
importantly) control the dialogue.

Use the “sandwich” technique: This is only a bit different from the
infamous “breaking bad news,” dialogue, but the sandwich technique is an
effective way to deliver constructive feedback and promote change in the
skills and behavior of the learner. To use the sandwich technique, first
reinforce and give positive feedback on skills and behaviors that you
directly observed. This fosters confidence in the learner, promotes positive
reinforcement, and usually encourages the learner to seek feedback from
others. After this step, ask the learner to identify deficiencies and areas
in which he or she thinks improvement is needed. If the learner has already
identified areas that need improvement (in the open-ended dialogue in step 2
above), you already taken the first step. Provide them with specific
examples on how they can improve and in the behavior/skills that you
directly observed that could be refined. When giving positive feedback, it
is best delivered immediately, while “on the spot.” When giving
constructive feedback, I’ve found it best to keep a log, and review
situations at a formal feedback session near the end of the learning
experience.

Seek confirmation and end with a plan: The goal of a constructive feedback
session is to be non-confrontational and to give the learner tools and
skills to facilitate change. At times, these sessions may be emotionally
charged, and the learner may take the feedback personally and feel “singled
out.” Following steps 1-3 above will help avoid this angst. Finally, have
the learner make an action plan before leaving the session. (E.g., exactly
how to approach a patient and care plan.) Have the learner give you their
summative assessment and then facilitate and form an action plan for moving
forward.

Molly 讲的内容很广泛, 我试着对其中对面试最有帮助的做些挑选, 肯定会有疏漏的
地方, 欢迎大家补充。 再次感谢Molly付出时间精力为大家做讲座, 并感谢叶子的组
织。

二月初或者一月底的rank meeting—faculty, current residents, admin staff会参加
会上讨论的内容—大家应该注意的
PPT上列出的内容 (包括有来interview 的人和来做过subintership的四年级的一学生
):
•    长相(便于大家记忆)
•    毕业学校
•    Score
(Molly 的program在一天中有六轮面试,秘书会把反馈的意见综合,放在PPT里。)讨
论时花在每个 candidate身上的时间是不等的,印象深的时间多些。讨论时interview
过 candidate的人会发言。大家关注的:
From candidate 1:
•    Geological tie
•    Candidate喜欢医院的patient population (i.e. for public hospital,
you might be seeing a lot of medicare and Medicaid patients 等等underserved
patient populations, 这点与那些well-off的neighberhood 中的hospital 是非常不
同的。)
•    Research experience, interested in fellowship application?注意一
般的community program 并不太在意这个,但Molly的program intends to set up a
fellowship program, so they do prefer candidate with research experience.
•    Do your answers during interview sound rehearsed? (有些对program
来说overqualified candidates把面试当成practice run, 所有的问题都对答如流,但
听起来不是很engaged)
From candidate 2
•    Coming from a DO program (对有些univ. hospital program 来说可能是
劣势)
•    Gap, but well explained
•    Good STEP scores, willing to start early and do a sub-i
•    Good hand—tied a perfect knot (对手术科室很重要)

Candidates are sorted into 3 categories:
•    Top tier—really would like them to come
•    Intermediate—like but maybe
•    Not likely to  consider (some bad experience during interview)
•    DNR (do not rank)—i.e. horrible performance during  Sub-
intership (不关注team work, 只想抢功劳)
What will shine during interview?
•    Local family tie
•    Engaged during pre-interview, interview and lunch/tour—什么叫
engaged, 准备好一些small talk 的问题, 在可能冷场的时候随时提出来—keep the
conversation going.
•    Personal connection with interviewer (Dinner is important, don’t
be shy, be nice, communicating with others as much as possible but do not
be talkative. ) Try to form bond with the interviewer by talking about
shared interests, etc.
•    对于specialty 有偏爱(i.e. 经历过别的specialty的residency, 但还是
喜欢OBGYN)
•    Have some talent that might be useful to the program (design,
photograpy, etc.)
In summary: good impression >no impression>bad impression,
Strategy:
•    “play to win”—if you don’t have  a lot of interviews
•    “play not to lose”—for young guys/gals, if you have a lot of
interviews
Note: interviewers does not always look for the same personality—so try to
be yourself and shine

To be valued, regardless of personalities:
•    Bottom line—finish your work
•    No dramas
•    Close all loops
•    Care about your surroundings (不止要把自己的事情做好,还要想想
team 需要什么)
•    Be a good communicator to patients, nurses,attending, consultants
from other specialties, seniors, juniors.
Things to talk about during interview:

•    An impressive question: “what surprised you when you started
your residency?”
Tips for small talks:
•    Mirroring
•    Make your point--在进行对话是表达你的意见,不要让主面试者觉得总是
他一个人在说。
•    Let them talk—keep the conversation going, do not change the
subject to you all the time.
•    Look at them in the eyes, smile, react to what they talk about

问题的种类:
•    让interviewer 比较happy的问题
•    自己想问题的问题
•    问得问题夹带想表现的自己的强项

不同background candidates的不同情况:
•    Fresh graduate (lots of interviews)

•    Peoples with Good scores should try to sell this
总的来说, 内科比较看重成绩,手术科室相对不是那么重视
Program selection—which programs to apply?

Post IV communication:
•    Thank you letter
•    Ranking: rank high or rank No. 1? (don’t say you are ranking the
program No.1 if you do not really intend to—Hurts your人品.
•    Second look? –less is more.  Your call.
